Based on observation, the licensee did not comply with the section cited above in that one child's medication is missing the expiration date or is incorrect which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on information obtained during interviews and record review, the licensee failed to comply with the regulation above as child C1’s authorized representative, per the facility’s Parent Handbook, was not notified of a hitting incident, which poses a potential health, safety or personal right risk to children in care. 8 future, the facility will adhere to all aspects of the Admission Agreement signed by parents, including Parent Handbook policies. Director agrees that any amendments to the Admissions Agreement be submitted to Licensing for approval.
Based on interviews, the LPA determined that facility staff neglected to outfit child C2 with supplied underwear necessary to meet the child’s personal needs, which poses a potential health, safety or personal rights risk to persons in care. 8 agenda and a signed/dated attendance sheet to LPA Poindexter by the
Site director Victoria Dedeaux stated that she had gone over the reporting requirements with staff during an all staff meeting on 9/13/23. Staff failed to notify parents of an injury to a child who sustained a bruise/open wound on the forehead. 8 All ouch reports are submitted to Area director for approval. Report is returned to Site director who will call parents regarding injury.
Based on observation, the Owls room was observed to be out of ratio with 20 children and 1 teacher and 2 aides. 8 Ms. Tapia shall submit a written plan of action to the department no later than Oct. 9, 2023.
Based on observation , the licensee did not comply with the section cited above as: Electrical outlet is uncovered, with wires accessible and is within reach of children, holes in wall adjacent hallway need filling in Ladybugs room and several large holes found in prescool playground, which posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above as five of 6 staff have no Health screening (staff 1, staff 2, staff 3, staff 4, staff 6), Staff 4 has no TB test in file, Staff 5 has no TDAP or employee rights, staff 6 has no staff file, which poses a potential health, safety or personal rights risk to persons in care.
Based on record review the licensee did not comply with the section cited above as four of five child files reviewed did not have physician's report (child1, child 2, child 3 and child 5) Child 5 did not have Health History or LIC995A Parents rights in file which poses a potential health, safety or personal rights risk to persons in care.
