(a) The licensee and the child's authorized representative shall jointly complete a current individual written admission agreement for the child... 1 Licensee will provide LPA a copy of the updated parent handbook. Based on LPA’s interviews and record reviews conducted on C1, C2, C3, and C4 did not have a current individual written admission agreement for review. This poses a potential health and safety risk to children in care. 8
Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidence by: Based on interviews and 1 Per director, S1 has been placed on administrative leave. Per director there was an in service meeting with all staff on 05/24/2024 reviewing supervison and personal rights. Director will submit sign in sheets of staff that attedned to LPA by POC date (06/07/2024). video footage review. S1 was observed to handle C1 in an inappropriate manner. S1 grabbed C1 by the arm and leg, lifted C1 and placed C1 on a cot. This poses an immediate health and safety risk for children in care. 8

Criminal Record Clearance: All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working...Request and be approved for a transfer of a criminal record exemption, ... the Department permits the individual to be employed... 1 Staff Lena Aceituno left the facility and director will submit all required forms to guardian for s1 clearance. This requirement was not met as evidence by: Based on LPAs observation and record reivew, Lena Aceituno was observed providing care and supervision to day care children and does not have clearance. This poses an immediate health and safety risk for children in care. 8
Based on LPAs observation and records review of child's #1 medication was expired on 09/2021, the licensee did not comply with the section cited above by discarding of medication by 09/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.
