Based on LPA’s interviews, 1 out of 6 staff disclosed C3 was bit on the leg by C4. C3’s parent came to S7. S7 admitted staff did not see the incident occur or realize at the time that C4 bit C3 on the leg. This poses an immediate risk to the health of the children.

101700.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 5.0 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. 1 Director made outlet D in "Room 3" inoperable by means of a sign that reads "Do Not Use" and covering the outlet. Outlet F is also inoperable by means of taping the area off and a sign that reads "Do Not Use". Director stated outlet D will be permanently removed. Outlet F will be replaced and re-sampled. Director will provide proof of replacement and re-sampled documentation via email by 10/14/22. Based on lab report analysis water from outlet D (18.00 parts per billion). Outlet F (5.8 parts per billion). Outlet samples were taken on 07/11/2022 indicate levels of lead in exceedance. 8 Filtered water pitchers from the other PASS sinks, along with individual bottle waters readily available for the staff and children.
