LPA observing child in an emply classroom on a table at 12:40pm on 2/26/25 without teachers knowledge. 1 Licensee will start her shift earlier in the day to oversee and train staff on supervision practices. She will also install door knob slip covers and a child safety gate on classroom doors and entrances. Licensee will send photos to clear the deficiency. This poses a potential health and safety risk to the children in care.
Based on LPA's observation a disinfectant spray bottle located in the daycare children’s restroom and accessible to daycare children. In addition, LPA observed other cleaning supplies located on the kitchen counter with the door opened which poses an immediate health and safety risk to the children in care. 8 The facility will submit this statement by 02/27/2025.
Based on LPA's observation three toddler aged children were commingling with the pre-school aged children in the pre-school classroom.
Personal Rights-The licensee shall ensure that each child is accorded the following personal rights...To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... The licensee terminated staff S1. The licensee said that she has already spoken with S1 about this type of behavior and was not going to allow her to speak to the children in this manner. This requirement was not met based on observation by LPA Ouye observing staff S1 yelling at toddlers which is a personal rights violation of the children in care. 8 LPA agreed that this plan of correction would be fine and that LPA could clear the deficiency if she decided to terminate S1's employement. Licensee email letter that S1 was terminated effective 4/25/24.
Based on interview with Director & Staff record review, the facility did not comply with the section cited above. Director could not provide proof of completion of the mandated reporter training for all staff, which poses a potential health, safety risk to persons in care.
