Based on interview and record review, the licensee did not comply with the section cited above in one of six staff files reviewed. Where S5 transcripts did not reflect six core ECE units. Which poses a potential health, safety or personal rights risk to persons in care.
Based on confidential interviews and a review of records, it was confirmed that facility staff did not meet the child 1 (C1) individual needs, resulting in Child 2 (C2) sustaining a fracture. The facility failed to follow C1’s behavior plan, which required close supervision to prevent incidents like this from occurring. This lack of supervision poses a potential risk to the safety and well-being of children in care.
Based on confidential interviews and record review, C1 was left unsupervised on the playground during a transition inside. Parent arrived in classroom to pick up child and child was found on the playground with complete absence of supervision. This poses a potential risk to children in care.
Based on confidential interviews and record review, staff were not properly supervising C1 and did not redirect C1 from climbing on top of a plastic bin. This resulted in C1 falling and fracturing their arm, requiring medical attention.
Based on interviews and record review there was a delay in CCC reporting the alleged inapproporiate incident between C1 and C2, which poses a potential, heath, safety or personal rights risk to children in care.
