Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Preschool director will provide a signed statements veirfyin that she and every staff are aware of Tittle 22 CCR reporting requirements and will fully follow the regulation. The signed statement will be emailed to german.negrete@dss.ca.gov by 07/24/2024. This requirement was not met due to LPA conducting staff and director interviews pertaining to a unusual incident, 02/24/2024 . Parent informed preschool that there child was being innapropriately touched by another child in the restroom. 8

a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 The director will provide a signed docuement verifying that she undertstands Tittle 22 California Code of Regulation section 101229(a)(1). And Director will email LPA a offical bathroom policy to german.negrete@dss.ca.gov by 07/24/2024 This requirement was not met due to LPA conducting staff Interviews. LPA found inconsitencies to the childrens bathroom break policy and the steps staff take to follow the guidelins. Also LPA obtained a signed declaration of how staff allow childen to walk to and from the bathroom unspupervised. 8

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.
