A Childs Village Preschool of Lompoc

Based on interview and record review, the licensee did not comply with the section cited above in two of three staff did not have copies of their transcripts which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in 7 out of 20 children were not signed in with either time times or parents signatures which poses a potential health, safety or personal rights risk to persons in care.

(i)Prior to employment or initial presence in the child care center, all employees and volunteers subject to a criminal record review shall: (1) Obtain a California clearance...(2) Request a transfer of a criminal record clearance. This requirement was not met as evidence by: 1 Director sent S2 home and understands that S2 must be associated and elgible cleared prior to S2 returning to the CCC. Director will submit a detail summary regarding communicating with Guardian and will ensure S2 is cleared and associated to the CCC prior to returning to the CCC. Detailed LPA's review of Guardian and LIS identified S2 is associated to the CCC but is currently reflecting "In Process" Per Director, S2 has been employed in the facility since about September 2023 and Director, was under the impression that S2 was cleared and associated to the CCC. 8 summary is due by 3/21/24 via email to Francisca.Velazquez@dss.ca.gov

Based on observation and interview the licensee did not comply with the section cited above in as LPA observed bowed plastic composite slates at the base of the tree which serves as a tripping hazard fron children in care and children in care are able to place their limbs in the opening between the slates. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview and record review, the LIcensee did not comply with the section cited above as S1, S2, S3, and S4's Mandated Reporter cerification expired and is not current, which poses a potential health, safety or personal rights risk to persons in care.
