(a) A center shall be equipped to isolate and care for any child who becomes ill during the day. 1 Center Director will conduct staff trainig on procedures for Isolation of Illness and document all staff in attendance and submit proof of training to LPA via email by agreed upon date Based on observation and interviews, the licensee did not ensure sick children were isolated from the other children in care, which poses a potential Health, Safety and/or Personal Rights risk to persons in care 8
(a) The licensee shall be responsible for ensuring that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. Center Director will create a health check log to use to screen chidren upon arrival in view of parents and submit proof to LPA via email by the agreed upon date. Based on observation and interviews, the licensee did not conduct daily health checks to ensure safe and healthful accommodations which poses a potential Health and Safety Personal Rights risk to persons in care 8
This requirement is not met as evidence by based on staff interviews and observations. Facility failed to maintain facility ratio. Staff confirmed morning hours between 6:30-AM-9:30AM the facility was operating over the ratio due to a lack of staff available to open the center. 1 Director immediately roated staff members to into coverage of the classrom (prek) and classroom (2's) to support with staffing ratios for nap time. Possible informal meeting with manager will be arranged to address ongoing ratio issues occuring at facility. Staff indicated that this happens frequently and almost everyday of the week. At 1:30 LPAS inspected the classroom (pre-k) during nap time and observed a classroom to have 1 staff with 22 children and 6 children awake and clasrroom (2's) to have 22 children with one staff and 2 children awake. 8
