a self report incident, 3 staffs' interviews, and reviewing documents that S1 left two 10 months old children unattended in the room for approximately 3 to 4 minutes in order to get a buggy out of the storage room away from the room during transitioning children to playground. This is an immediate risk to the health and safety of children in care. 8 The statement will be sent to LPA's email address by the due date of 1/10/2025
