admission, review of footage, and through interviews. Former staff were 1 Both Staff 1 and Staff 2 were placed on leave and investigated when these incidents. As a result of the investigation the staff are no longer employed at the facility. Director will conduct a training regarding staff interactions with children and personal rights. Proof of observed placing a child in their seat in a rough manner and pushing them toward the table. The same staff was also observed moving children in a manner that was inappropriate by grabbing them by their jacks or sweaters. Another former staff was also observed pulling a child up by one arm in a manner harmful to the child. This posed an immeidate risk to the health and safety of chidlren in care. 8 the training will be sent to LPA 05/21/2025.

Food Services. (14)All persons engaged in food preparation and service shall observe personal hygiene and food- service sanitation practices that protect food from contamination. This requirement was not med as evidenced through admission. On 04/08/2025, Staff 3 was observed 1 Administration spoke to Staff 3 and reminded them of food services and proper hygiene. Director will conduct a staff training that will include food services and hygiene. Proof of the training shall be sent to LPA by 05/21/2025. feeding themselves a food item with their fingers and then proceeded to serve children food without washing their hands or putting on gloves. If not corrected, this poses a potential risk to the health and safety of children in care. 8

interviewes. It was conclusive that mutiple children on more than once occassion were picked up by their guardian 1 LPA and director discussed the possibilty of having another teacher in the Early Preschool Classroom designated for diaper changes. A plan will be developed to ensure that diaper changes occur in the afternoon - early evening before pickup. Facility will notify parents of diaper rashes and ask for with a full diaper. Evidence was also conclusive that multiple were observed to have diaper rash after being picked up from the facility. This poses a potential risk to the health and safety of children in care if not corrected. 8 them to bring the appropriate diaper cream. A formal written plan shall be submitted to LPA by 05/16/2025.

It was confirmed that a staff supervised more than 1 Licensing representative has agreed to the following: 1) two more teachers will be scheduled to relieve the opening room at 7:30 and 7:45 am. Proof of this schedule change will be sent to LPA by 02/26/2025, 2) proof of staff adhering to the schedule change will be sent to LPA by the close of business on 12 children for a period of at least 15 minutes by themselves in the opening room. It was also confirmed that staff have previously supervised more than 12 children alone on multiple occassions other than this morning. This is considered an immediate risk to the health and safety of children in care. 8 02/26/2025, 3) facility will review sign in logs from 7:00-8:00 am from previous weeks to ensure that the schedule change will accomodate the ratios necessary 4) Facility is currently hiring for 2 vacant positions 5) LPA will conduct a plan of correction visit.

interviews during a complaint investigation and an unusual incident report that was 1 Staff #3 is no longer employed at the facility. Director stated that she will review with staff procedures for safety checks of the playground before usage by children. Director will reiterate to staff that smoking is not allowed on or near the premises. reported to LPA via email on 12/21/22. On 12/15/22, Child #1 was found with a vape pen in her mouth by Staff #1 and Staff #2, after it was left on the playground from Staff #3. Evidence was also conclusive that Staff #3 previously used the vape pen in the staff break room on site. This posed an immediate risk to health and safety of children in care.
