Based on observation, the licensee did not comply with the section cited above as LPA observed the preschool play structure does not have a United States Consumer Product Safety Commission (CPSC) label or a manufacturer’s label indicating the age-appropriate usage. which poses a potential health, safety or personal rights risk to persons in care.

Based on records reviewed and interviews, Staff 1 handled day care child in a rough manner on 03/04/2025. Also, it was reported other children were being mishandled in a rough manner and yelled at by Staff 1, which poses an immediate health, safety, or personal rights to children in care.

Based on record review, the facility did not comply with the section cited above. Three of the 5 teachers present have not completed and/or renewed the required Mandated Reporter Training (MRT) which poses a potential health, safety or personal rights risk to persons in care.
