Director will also create a written statement on how this violation has been addressed with the staff member (S5) and a plan for imporvement which will be signed by both parties. Email these completed statements to LPA Uribe by the due date of 04/10/26.

Personal Rights. The licensee shall ensure that each child is accorded the following personal rights:To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. 1 Director is to conducted an all staff meeting. Diapering procedures/policies must be discussed including the importance of changing diapers once soiled. Meeting agenda and signatures of attendees to be emailed to LPA no later than 11/27/2023. This requirement is not being met as evidence by: Interviews conducted stated that there has been a time(s) when their child has come home with a soiled diaper resulting from child not being changed in a timely manner. This poses a potential risk to the health and safety to the children in care. 8 **Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.**

interview: A staff member was speaking in an inappropriate way to a child in care. CCL regulations state that children have personal rights, and those rights include to be treated with dignity in his/her personal relationship with staff and other persons, and to be free of mental abuse. This poses an immediate health risk to the Personal Rights of children in care. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Section 101229 (a)(1) that no child(ren) shall be left without the supervision, including visual supervision, of a teacher at any time, except as specified in sections 101216.2 (e)(1) and 101230 (c)(1). 1 The Director conducted an internal investigation regarding the incident on 7/7/2023 and a Staff Meeting on 7/21/2023 focusing on CRC retraining and play yard. Based on the Staff Interview, a child was left unattended after the class transitioned from the outdoor play area to their classroom on 7/7/23 at around 4:15 to 4:35 pm which was an immediate risk to the health and safety and personal rights of children in care. 8 LPA recommended CDSS CCLD Technical Support Program (TSP) to the Director and provided a TSP brochure.
