Based on observation and record review, the licensee did not comply with the section cited above in Disaster drills were not documented which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in staff members not having lic 503 and proof of immunizaions or flu declinations which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on the interview and record review, the Center did not comply with the above regulation. A staff member pulled C1’s hair to demonstrate how it felt when C1 pulled others’ hair. This intention violated the child’s personal rights, which poses a potential health and safety risk to the children in care. 8 As part of the plan of correction, the center will submit all training materials and the teachers' attendance confirmations to the Licensing Office. Once received, the deficiency will be cleared.

Based on results received from the certified water samplers, the facility exceeded 5.4 ppb of lead in their water source. Once received, the facility took immediate and proper actions to remediate the fixtures. This poses a potential health, safety, or personal rights risk to persons in care. 8 The director will update the LPA for the retest schedule and results will be forwarded once available.
