A daycare child, C1, exited the playground unnoticed by staff and had gone out the gate. This poses an immediate risk to the health, safety, and personal rights of children in care. 8 Because the facility has been cited for repeating the same violation within 12 months, an immediate civil penalty of $1000 was assessed. Civil penalty of $100 per pay continues until the violation is corrected.
Based on interview and record review, the licensee did not comply with the section cited above as one staff member did not have proof of immunity to measles or pertussis, which poses a potential health, safety or personal rights risk to persons in care.
Based on interview and record review, the licensee did not comply with the section cited above as two (2) out of five (5) children's files did not have verification of receipt (LIC9224), which poses a potential health, safety or personal rights risk to persons in care.
Since the incident occurred, the Site Director has met with all staff members regarding the importance of headcounts, utilizing walkie talkies, and giving children reminders prior to transitions. The Licensee did not ensure that children are visually supervised at all times as a child was left outside during a transition to inside, which poses an immediate risk to the health, safety, and personal rights of children in care.
The Site Director has relocated the sign-in/out for the center, so parents come into the classroom and do not prop the door open. She further states that parents have been reminded to keep the classroom door closed. The Licensee did not ensure that children are supervised at all times. During pick-up time, a child's whereabouts were unknown and the child was located outside of the facility, which poses an immediate risk to the health, safety, and personal rights of children in care.
The Licensee will submit written report for the incident that occurred on 11/14/2023 and submit to the Department by 11/29/2023. The Licensee did not notify the Department or submit a written report regarding a absense of supervision for a child on 11/14/2023, which poses a potential risk to the health, safety, and personal rights of children in care.
The Preschool Program Director present at the facility was used as a fully qualified teacher even though she was missing child/family/community within the core twelve semester units which poses a potential risk to the health, safety, and personal rights of children in care. 8 Copy of updated hiring requirements for Preschool Program Director will be submitted indicating the core twelve units for fully qualified teacher to the Department by 11/18/2022. Position should also highlight on experience needed as well as steps to obtain an exception request if needed.
