c(1)(A) (c) To be a fully qualified teacher... Twelve semester units.passing grades, (A)shall include courses...of child growth and development; child, family and community; and program/curriculum. This requirement was not met evidenced by: Per director, assistant teacher has 9 current units and is waitlisted for the missing child development course. Director stated she teaches CHDV at Chaffee and will ensure to enroll assistant teacher into needed CHDV course. Director will email confirmed enrollment to LPA M Ruiz by 10/13/2025. Based on observation and record review, 2 assistant teachers were left alone supervising 11 two-year olds without a supervising qualified teacher, which poses an immdeiate health and safety risk to children in care. 8

(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirment was not met as evidence by: 1 Per director the drinking fountain water fixture"I" with lead exceedance was capped off on 03/31/2023. LPA took photo of the capped off fountain. Children bring water from home and are provided refills from bottled water. Based on record review, the facility had a high lead exeedance of 11.00 UG/L on drinking fountain fixture "I" that poses a potiental health and safety risk to children in care. 8
