(e)In centers where the licensee chooses to handle medications:(5)The licensee shall develop and implement a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given. 1 Licensee stated that she is going to write up a written plan on how medication will be handled at the facility, including specific forms to be used, how to use them and will train staff on the written plan and how to use the forms and how to inform child's authorized representative when administering medication to children. Licensee will send written plan and sign in sheet for staff trained to LPA Strother by 09/01/22. A review of C1's records indicates that the administration of medication given to C1 on 08/16/22 was not recorded. Staff interviewed had no knowledge of a written plan to do so, which poses a potential risk to the children in care. 8

Licensee stated that she will submit a written Unusual Incident Report to the Department regarding the incident that occurred on 08/16/22 were a child was given medication and had side effects observed by staff. The facility was made aware that C1 was given a additional dose of a 12 hour medication that C1 had received prior to arrival and side effects of the overdose of medication were observed by Staff and C1's parent. A review of records indicated that this incident was not reported to the Department as required, which poses a potential health, safety and personal rights risk to the children in care.

L1 stated that she has taken action by terminating S1, hosted a training on the topic of Personal Rights for all currently employed staff and has given each current staff a copy of the Title 22 Personal Rights regulations. By 08/16/22 L1 will create and submit to LPA Strother a written plan (to include date, time On 07/19/22 S1 picked up C1 and carried her to the hand washing line, scratching C1’s underarm, leaving a mark. It was reported by L1 that while C1 was crying, S1 told C1, “Waa, Waa, Waa, stop your crying”, which poses an immediate Health and Safety risk to children in care. and persons who will attend) a staff training to discuss techniques to use for smooth transitions with children. L1 will have staff sign in as documentation that they have completed the training, submitting the sign-in sheet to LPA Strother by 08/29/22.
