Based on observations, interviews and record review, the facility did not ensure children in care were not bitten by another child in care, which posed a potential Health, Safety or Personal Rights risks to persons in care. A specialist was consulted with for recommendations. Sensory items and other types of toys have been introducted into the classrooms. Resources such as books have been provided to staff.

Based on interview with Director, Staff #1 was hired and allowed to work before the background clearance was completed. This is an immediate risk to the health and safety of children in care. 8 She will ensure all future hires have a completed clearance prior to hire.

Based on interview with Director and Staff #2, Staff #1 stepped away from a preschool child on the changing table, for approximately 10 seconds, on twodifferent occasions. This is a potential hazard to the health and safety of children in care. 8 every changing table in the faciltiy. A copy was provided for LPA at this visit. She will ensure that staff adhere to this policy and there will be a reminder at staff meetings or more often, if needed.

Based on record review, the licensee did not comply with the section cited above in that Staff #1 and 2 did not have the Measles vaccination information on file and Staff #3 was missing Measles and Pertussis information which poses a potential health, safety or personal rights risk to children in care.

Staff did not prevent daycare child from biting multiple daycare children resulting in injury(ries).
