Based on interviews and a complete review of the incident, it was determined that facility staff failed to ensure that (C1) was under the direct supervision of a teacher at all times which posed an immediate health and/or safety risk to C1 and children in care. 8 3.Professional Development Training Due to the severity of the incident, the facility and the organization will receive a formal letter requesting an informal conference meeting. This meeting will be scheduled in the near future.
