Based on results received from the certified water samplers, the facility exceeded 5.4 ppb of lead in their water source. This posed a potential health, safety, or personal rights risk to persons in care. 8 Deficiency will be cited and cleared on the same visit, 10/26/2023.

Based on record review and medication review the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. One child Epi Pen Jr. has been expired. Another child has been prescribed Epi Pen Jr. but does not have Epi Pen Jr. on site.
