101229(a)(1) Responsibility for Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement was not met by: The pastor stated door knob covers are in place. The doors to the main lobby will be closed and lock. The pastor stated he will create a plan and submit the plan to LPA Hood within 24 hours. Based on interviews a child was found outside of the facility by a female and daycare parent. Due to facility doors being left open for facility ventiliation a child left the facility. This is a immediate Health and Safety Risk to children in care. 8 Request Denied
101212 (d)(1)(C)• Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events…a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information…shall be submitted to the Department within seven days following the occurrence of such event. Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met by: 1 The pastor stated he will complete the LIC 624B and submit the form to LPA Hood. The pastor will create a plan to ensure incidents are reported to the Dept. with 24 hours and the LIC 624B is submitted within seven days from the date of the incident. Based on the admittance of Pastor John Roamer and a file review completed by LPA Hood, the facility did not notify the Dept. within twenty-four hours or submit the LIC 624B (UIR report) within seven days from the day of the incident. Based on LPA observations and interviews conducted, the licensee did not comply with the section cited above, as staff did not maintain visual observation of children inside the restroom, which poses an immediate Health and Safety risk to children in care.
Based on observation, the licensee did not comply with the section cited above in not having direct supervision on child that was in the bathroom alone, which poses an immediate health, safety or personal rights risk to persons in care.
The Facility Representative, Pastor John Roamer and the Director Ms. Prysock stated the two children, who were biting, have been moved to an older preschool classroom with a verbal plan in place to shadow the two children and provide one on one by the teacher. LPA’s confidential interviews and the evidence obtained; two children were bitten by another child resulting in an injury. The two children sustained bite marks which broke the skin. This poses an immediate health and safety risk to children in care. 8 The two children have not bitten another child since they put the verbal plan in place with child’s parents and staff. The appeal rights were discussed and provided.

As per the evidence and confidential interviews; the same two day-care children have bitten multiple children in care on several different occasions. There were at least seven different biting incidents in a short span of time. This poses a potential health and safety risk to children in care. 8 The two children have not bitten another child since they put the verbal plan in place with child’s parents and staff. The appeal rights were discussed and provided.
Based on observations and interviews, staff stated five children were in care displaying a runny nose. On 10/21/2021, LPA observed 14 children and one staff without a face covering in classroom #2. LPA observed a child in care displaying a runny nose. On 10/27/2021, LPA and LPM observed three children in care displaying a runny nose. Staff were observed with face coverings below the nose or under the chin. This poses an immediate health, safety and personal rights risk to children in care.
Based on undetermined dates, at least two staff observed two staff telling children they were busy in a demeaning and disrespectful voice tone. Another staff was heard making humiliating comments towards a child in care. This poses a potential Health and Safety risk to the children in care.
Based on LPA and LPM's observations and interviews, the facility is not enforcing the face covering requirement per the (CDPH) for children 2 years and older. This is a potential hazard to children in care.
