101416.5 Staff-Infant Ratio (b) There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met evidenced by: 1 Director stated that she will submit a statement of understanding of the regulation and detailed plan to ensure ratio are being met, to LPA by due date. Based on LPAs interviews and record review, S#11 did not meet qualification to work in the infant classroom and had worked in the infant classrooms on multiple occasions, this posed an immediate risk to the health, safety and personal rights of the children in care 8

101416.2 Infant Care Teacher Qualifications and Duties (b) Prior to employment, an infant care teacher shall have completed... units related to the care of infants, at an accredited or approved college or university. This requirement is not met evidenced by: 1 Director stated that she will submit a statement of understanding of the regulation and detailed plan to ensure staffing qualifications are met. Based on LPAs interviews and record review, S#11 did not complete 3 units in Toddler/Infant care prior to working in the infant classroom, this posed a potential risk to the health, safety and personal rights of the children in care. 8

101419(a)(2)(C) Documentation shall be maintained in the infant’s file... include the following: 1.Date. 2.Infant’s name. 3.Time of each 15-minute check. 4.Initials of staff person who conducted each check. This requiremnet is not met evidenced by: 1 Director stated she will submit proof of Sleep logs for the infants in care to LPA by by due date. Based on interview and record review, Staff Staff 1 and Staff 2 admitted that facility did not complete Sleep log ofr 4 infants in care, which poses a potential risk health, safety and personal rights for the children in care. 8

Based on interview and record review, the Director admitted that the facilty had not completed the lead testing requirment for the center as of 03/10/2023, facility did not comply with the section cited above, which poses a potential health and personal rights risk to persons in care.

Based on interview and record review, the Director admitted that staff did not comply with the section cited above in 4 out of 4 infant's files which poses a potential health, safety or personal rights risk to persons in care.

Staff do not document sleep checks for infants in care.
