Owner Hamid was advised that employees cannot be present in the facility unless a criminal record clearance has been received by the Department. He states the employees had started the fingerprint process but have not yet received a clearance. To correct the deficiency, owner shall submit a clearance for the two eployees. Two staff were observed working in the facility today supervising children and used in ratio. (see LIC811 for name). The two staff did not have a criminal record clearance. This an immediate risk to the health and safety of children in care. Civil Penalties are assessed in the amount of $500.
LPA will make a return visit to verify correction has been made. LPA Blesi observed one staff supervising 29 napping preschool children putting the room over ratio.
Director shall obtain a criminal record clearance for S2 before the employee can work or be present in the facility. Proof of correction shall be sent to LPA by 1.06.22. S2 was observed working in the facility today supervising children and used in ratio. LPA conduced a file review and Guardian search and it was determined a fingerprint clearance was not located for S2. (see LIC811 for name). S2 has been employed since 9/27/21. This is an immediate risk to the health and safety of children in care. Civil Penalties are assessed in the amount of $500.
LPA reviewed the ratio of the toddler component (6:1) with director. Director immediately stepped into the toddler room to meet ratio until another teacher arrived. Deficiency cleared during inspection. LPA observed S1 in the toddler room supervising 7 toddlers. This poses an immediate risk to the health and safety of children in care.
the Redwood classroom had 24 children supervised by an aide (S2) and a FQT (S3). LPA observed an aide (S4) and a FQT (S1) supervising 29 children on the playground. Both the classroom and the playground were out of ratio. This poses a risk to the health and safety of children in care if not corrected.
