Based on a review of records, the licensee was found to be noncompliant with the section cited above, as 3 out of 5 staff files reviewed by the LPA did not contain valid mandated reporter training certificates. This poses a potential risk to the health, safety, or personal rights of persons in care.

Based on record review, the licensee did not comply with the section cited above because one teacher (S5) does not have health screening form and Tdap record which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above because one child (C6) does not have an immunization record on file which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above because two medications do not have consent in writing from children's authorized representatives which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above because three medications did not have children's names and were not in original packaging which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above because PUB 393 Notification of Parents' Rights, Waiver, LIC 613A Personal Rights and LIC 610 Emergency Disaster Plan were not posted in the center which poses a potential health, safety or personal rights risk to persons in care.

During today's inspection LPA Huang obtained the following documents; Self-Certification LIC9275, Sampling Checklist Form LIC9276, Facility Sketch LIC 999, fully labeled with locations of all water outlets, and full lead report. This exceedance level found in the play yard drinking fountin was 45ppb. This poses a potential risk to the Health, Safety, or Personal Rights of children in care. 8 The drinking water fountain located in playground was covered by plastic paper and no longer in use. The facility to fix the lead exceedance, and pass the test or to remove the fountain.

Based o record review, the facility did not comply with the section cited above as two children were missing their physician' reports.which poses/posed a potential health, safety or personal rights risk to persons in care.
