LPA observed C1 1 Licensee advised LPA facility will ensure parents are aware of the facility procedures Due Date: 5/20/26 and C2 without teacher supervision on 05/20/26
At approximately 1:38 pm LPA Laureano observed the two-year-old classroom with 17 sleeping preschoolers and two children who were awake with one teacher. Citations was corrected by the end of the visit.
During the complaint investigation interview, it was revealed that Staff 2 yelled at children in care. 8 Director will complete and submit a signed declaration stating that S2 is no longer at the facility. Declaration was collected during the inspection.
Based on observation, interview and record review, facility staff were unaware of the injuries sustained by C1 while in care, which poses a potential Health or Safety, or personal rights risk to persons in care. 8 Director will provide copy of memo with staff signatures indicating that they have received memo.
(n) Furniture and equipment shall be maintained in good condition, free of sharp, loose or pointed parts. Since the incident occurred that table has been removed and no longer present at the facility. Type A deficiency cleared during today's visit. Based on interviews and information received this standard was not met by evidence of Administrator verbal statement and provided on a declaration that a table at the facility was not free from sharp items and that a child sustained an injury from the table. 8
(d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours... Type A deficiency cleared today by a written declaration from Administrator stating that she understands the regulation mentioned and will maintain compliance of reporting requirements in accordance to title 22. A copy of the regulation was provided to Administrator during today's inspection. Based on LPA's interviews and information received this standard was not met by evidence of Administrator admitting that an injury incident occurred at the facility on 11/19/2021 and was not reported to the El Segundo Regional office. 8
