101238 Buildings and Grounds (a) the child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees, and visitors. This requirement is not met as evidence by: LPA observed facility entrance gate in good repair during visit. Director stated she will ensure facility gate is always in good repair. Based on interviews conducted. It has been determined that the facility gate that is located between the children’s playground and the facility parking lot was in disrepair. This poses potential Health and Safety risk to children in care. 8
