During LPA interviews with staff and Licensee, it was reported that on 03/13/2026, 2 (two) children in care hit one another in the face with a frisbee and one of the children sustained a black eye. The incident was not reported to the Department as directed in the above Regulation, which poses a potential Health and Safety or Personal Rights risk to children in care. 8 In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.

101700.3(b)(1), A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. 1 Facility has made faucets/outlets, identified with an Action Lead Exceedance of 5.5 ppb or over inaccessible for use until a retest has occurred. Director will email LPA a copy of parent notification letter via email by 12/8/2023. Director will send LPA a copy of schedule repairs and re-test summary. Based on observation, interviews, and review of lead water testing samples taken on 09/16/2023 revealed that levels of lead were beyond allowable amount deemed by the State of California-5 water outlets identified (B and C). This poses a potential health and safety risk to children in care. 8

(a) The licensee shall ensure that each child is accorded the following personal rights: ... (2) To be accorded safe, healthful and comfortable accommodations,furnishings and equipment to meet his/her needs. Licensee agrees to make the area of the plastic play equipment inacessbible, remove the children's bike without pedals or handles bars and remove the indoor storage bin. LPA observed director remove all items. This requierment was not met by: 2 outdoor toys: children's bike and plastic play equipment and 1 indoor toy bin was observed with visible broken pieces. 8 Director agrees to remove the children's bicycle and toy bin. The outdoor plastic equipment was taped until it can be discarded.
