Based on evidence obtained, the facility was operating out of ratio in the 2's (Homeroom) at early drop off which poses a potential health, safety or personal rights risk to persons in care.

Based on the self reported incident the facility was operating out of ratio on 3/19/25 when S1 was left by herself on the playground with 27 children which poses a potential health, safety or personal rights risk to persons in care.

Based on staff interviews the facility failed to provide visual supervision resulting in a child was left alone in the classroom.

Based on record review, the licensee did not comply with the section cited above in 5 of 52 children present were not signed into the facility by a parent/guardian. This poses a potential health, safety or personal rights risk to persons in care.

The director states that she conducted training on transitions and supervision with all current staff. The Director provided a copy of the training agenda and staff sign in sheet for the staff training held on 10/11/21. The director will submit a signed and dated statement explaining how she will ensure that staff are following the correct procedures for transitions/supervision.
