101212 Reporting Requirements (a) Each licensee or applicant shall furnish to the Department reports...(d)Upon the occurrence, during the operation....(1) Events reported shall include the following: (B) Any injury to any child that requires medical treatment. 1 Director will view the Child Care Reporting Requirements video and provide a document regarding what you learned from the video. Licensee will send information by email or mail to LPA by POC. This requirement was not met as evidence by: Based on Licensee statements. Licensee stated she did not see an injury on child to report to CCLD nor did she feel it was necessary to report the injury to C1's parents as child seemed fine. 8
