Based on interviews and record review, the licensee did not comply with the section cited above in when C1 was pulling away from S1 his elbow was dislocated which poses/posed a immediate health, safety or personal rights risk to persons in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Based on Lead Testing Sample results the facility has one outdoor water fountain with lead exposure. Director stated children used it for drinking water until March 2020 and not been used since then. Since children were exposed to water with lead, this posed a potential risk to Health and Safety.
