Based on 3 of 4 interviews C1 has been sent home in soiled clothing on multiple occasions. One one occasion C1 was brought back to the facility after being picked up to show evidence of C1 being soiled.
Based on record review, the licensee did not comply with the section cited above as two staff S2 and S3 did not have current up to date mandated reporter certification which poses a potential health, safety or personal rights risk to persons in care.
Facility has replaced faucets M & N and had them retested and re-labeled A & B and passed. LPAs received the facilities water testing results for drinking fountains D, E & F with an Action Level Exceedance higher than the allowable limit.
