Based on staff interviewed, a 5-month old infant was allowed to sleep on the floor(daily) due to the infant's difficulty in sleeping. According to staff the infant will awake as soon as they try to lay infant in the crib. 8 Director will submit a copy of the plan to the department no later than 5/5/2025.
