Based on interviews with the Director, Vanessa Milroy, staff memebers (S1-S3) and documents obtained , an infant child was left alone inside a closet between Classroom 1 and 2 for approximatley 5 minutes without supervision on 06/13/2025.
Based on interview with Director, Staff S1 did not ensure supervision of an infant which poses an immediate Health and Safety risks to persons in care. Director will submit sign sign out sheet for this meeting to LPA on 1/31/25.
Personal Rights - (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Director states that they will impliment a Links to Learning Program Leader to ensure that staff are continuously meeting regulation requirements. Director will provide the name of new Program Leader and training list by POC date via fax or email.. Based on LPA's observations and record review, the licensee did not ensure that child care equipment was being used according to the manufacturer's instructions. This poses a potential health & safety risk to children in care. 8
Based on water testing results and interviews, facility tested over the Action Level Exceedance level at on water fountain in Room 2. This poses an immediate/potential health, safety or personal rights risk to children in care.
