Per Director, the LIC 9182 form was faxed today to the Regional Office. Director understands that employees fingerprints should be transfered prior to working in the facility. Per Director, she confirmed with the Regional Office that S1 has been associated. LPA observed Staff 1 (S1) is not associated to the facility. LPA observed on LIS that S1 has a fingerprint clearance. Per Director, she didn't submit the transfer request form. This poses an immediate risk to the health, safety, and personal rights of child in care.
Based on interview, the licensee did not comply with the section cited above in outlets use for drinking and food preparation has not been tested for lead as required which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in director and staff have current certificate, but the certificate is from an entitly that is not EMSA approved. No EMSA sticker on certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on interview and record review, it was disclosed by 3 of 7 staff, that a child was left unattended in a classroom without supervision on 11/19/2021. This posed an immediate risk to the health, safety, and personal rights of child in care.
