101229(a)(1) Responsibility for Providing Care & Supervision: No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met evidenced by: 1 The Director states she has provided training for their staff regarding care and supervision strategies for the children in care, and continued to train staffs. The director submitted proof of training to LPA during the visit. Based on interviews, five (5) interviewed staff members who were providing supervision for the children in the facility in the classroom on 04/04/2025 confirmed that they were not aware of the incident in which C1 exhibited inappropriate behaviors with other children in the class. This poses a potential risk to the health, safety, and personal rights of the children in care. 8
