... 1 The Director, Ms. Castaneda, will provide copies of all LIC9224 acknowledgement forms (related to the 6/17/25 licensing report) to parents of the facility and provide a copy of the facility roster to LPA Poindexter by 9/18/25. Based upon interviews and record review, the facility delayed the required LIC9099 licensing report dissemination and collection of LIC9224 forms, which provide proof that licensing reports were received. This is a potential health, safety and personal rights risk to children in care. 8 Director may submit these documents to LPA Poindexter at 7575 Metropolitan Drive, Suite 110, San Diego CA 92108. Director will also submit a written, dated and signed statement that, in the future, they will provide copies of any Type A citation and LIC9224 forms to parents, as per regulation.
