Based on observation, the licensee did not comply with the section cited above. LPA obsrved 2/3 infants siged into the facility which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above. Infants under 2 did not have documented 15 minute sleep checks which poses/posed a potential health, safety or personal rights risk to persons in care.
LPA confirmed that the facility has shut off water to the outlet and observed caution tape around it to prohibit it being used. Director advised that if there is still an exceedance after retesting, they will replace the outlet and provide LPA with proof. Lead test results received by Licensing on 2/7/2023, tested on 1/14/2023, showed an outlet in the facility had lead levels exceeding 5 parts per billion (ppb). This poses a potential risk to the health and safety of children in care.
