Based on interviews and review of records it was observed that facility allowed teachers aides to be left alone with children as well as had a "volunteer" step into ratio. This posed an immediate risk to health and safety.
Director stated that staff will informed and trained to not allow personal food to be fed to children, director to send documentation of training and keep signed copies in staff files. Based off of interview of staff and management it was stated that staff did share meal with child while in care. This poses a potential risk to children in care.
Based on record review, the licensee did not comply with the section cited above in 2 out of 7 staff were missing documentation of immunization which poses a potential health, safety or personal rights risk to persons in care.
