Based on record review, the facility had faucet A with lead test results exceeding 5 ppb of lead in the water. This is a potential health and safety risk to children in care.

(a) Certified external water samplers shall be used to conduct the Child Care Center water collection 1 Licensee agrees to have certified external water sampler complete lead testing at facility and have results uploaded to database.The licensee agrees to submit an LIC 9275 and LIC 9276 by 5/18/24 Based on record review,LPA reviewed that lead testing was completed 8/4/22 but facility did not use a certified external water sampler. 8

To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. 1 Licensee will establish more of an open communication with staff, families and parents. All parents should have a signed LIC 9224 in children's file for new and currently enrolled children. Licensee will submit a written plan of correction to CCLD by 6/30/23. but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This was not met as evidence by: based on interviews, children were handled roughly by staff. This poses an immediate health, safety, or personal rights risk to persons in care.
