(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual obs.. 1 Per Licensee, they will continue to come every day- arriving at opening time- to close time. Currently, interviewing for additional staff. Licensee, will write a decleration with written plan. This requirement is not met as evidence by: Based on observation and interview the Licensee did not comply with the section cited above in that S2 has been the only teacher in the classroom and is not providing direct and visual supervison to the children-- see continuation to the right------- 8 when they step into the kitchen, bathroom and children are going to the bathroom by themselves.
Based on observation, the licensee did not comply with the section cited above in that carbon monoxide detector was on the kitchen floor and it did not have any batteries, which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that there was no documentaion available for review. Per staff # 1 drill was conducted two months ago but they do not document which poses/posed a potential health, safety or personal rights risk to persons in care.
