Based on observation, and record review, the licensee did not comply with the section cited above in two counts out of total of 3 counts of staffs' fingerprinting which poses a potential health, and safety risk to persons in care. Grace Leonard and Marilyn Sorense did not have fingerprinting clearance through Department of Social Services. Their clearances were through Boys and Girls Club which is a different entity. LPA observed they were supervising children. 500.00 civil penalty was assessed for each staff today. (total of two staff).

Based on observation and record review, the licensee did not comply with the section cited above in 3 counts out of total of 3 staffs' files which poses a potential health, and safety risk to persons in care. Staffs' files were not available for LPA to be reviewed. (Total of 3 staff).

Based on interview and record review, the licensee did not comply with the section cited above by not testing their water for lead as it was required which poses a potential health, safety risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 2 counts out of 3 files that missing Mandadter Reporter Training Certificates for staff # 2 and staff # 3 which poses a potential health, safety risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 2 counts out of 3 files that missing required vaccinations for staff # 2 and staff # 3 which poses a potential health, safety risk to persons in care. Staff # 2 missing MMR and Dtap. Staff # 1 missing Dtap vaccines.

Based on staff files reviewing today at 10:00 am, the licensee did not comply with the section cited above in 2 staff out of 3 staff file reviewing, which poses a potential health, safety risk to persons in care. Staff # 1 and one staff # 3 present in the classroom were missing the above documents on file.
