Based on observation, the licensee did not comply with the section cited above when the facility did not have a Carbon Monoxide detector to test, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above when children's files did not contain signed LIC 9224 documents from a 10/05/2023 Type A citation, which poses/posed a potential health, safety or personal rights risk to persons in care.

All preschool staff will sign a statement acknowledging that they have watched the videos titled “Teacher-to-Child Ratios in Child Care Centers” and “Supervising Children in Child Care Centers” available on the CDSS website. one teacher supervising more than 12 children in attendance based on observation, interview, and record review. The Licensee did not comply with the above regulation which poses an immediate health, safety, and personal rights risk to children in care. LPA will be emailed the statement(s) with all staff signatures by 5 PM on 10/13 /2023. LPA will conduct an unannounced POC visit to clear the deficiency.

During today's visit, LPA observed that the affected water outlets are not in use. D1 will schedule re-testing and submit the water report to the RO to resume using these 5 water outlets. Deficiency will be cleared during today's visit. water report revealed that the facility had elevated levels of lead in 5 water outlets. This is a potential health and safety risk to the children in care.
