Licensee Representative states that she will submit a written plan by 11/21/25 to ensure that there is a fully qualified teacher in the classroom at all times. LPA observed a teacher's aide (S3) supersiving six preschool children in Room 4 which poses an immediate health, safety or personal rights risk to persons in care. 8 Because the facility has been cited for repeating the same violation within 12 months, an immediate civil penalty of $250 is hereby assessed. An ongoing civil penalty of $100 per day will be assessed until the violation is corrected.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 staff. Staff #1 & #2 are missing Mandated Reporter Training (AB1207) certificate which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 staff. Staff #2 & #3 are missing immunizations in measles and pertussis which pose a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above by not having a qualified Site Director which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 3 out of 3 staff. All the three staff members who are working at the center are not qualified preschool teachers which poses a potential health, safety or personal rights risk to persons in care.

Based on LPA's observation and record review, the licensee did not comply with the section cited above. S1 & S2 who were present in Room 4 supervising five (5) preschool children are missing units to be a fully qualified preschool teachers which poses a potential health, safety or personal rights risk to persons in care.
