101700.3 California Lead Action Level at Child Care CentersTesting r (b)results with fractional ppb readings of 5.0 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was met as evidency by 1 Director stated that parts have been ordered and will be replaced or capped off and removed. Oonce retest results are obtained, results will be emailed to LPA. Faucets with lead exceedance are outdoor hose spigots and are not used for food prep or consumption. Plan of Correction Date of 07/31/2023. Lead water testing samples taken on 10/20/2022 and retested on 02/14/2023 revealed that levels of lead were beyond allowable amount deemed by the State of California. Faucet N lead exceedance level tested at 7.7 ug/L and Faucet J lead exceedance tested at 6.6. This poses a potential health and safety risk to children in care. 8

On 01/10/2023 child #1 was left unattended inside of the classroom approximately for 15 minutes. Parent of child#2 found child#1 inside of the classroom and reported to Staff#1 and Staff#2, both Staff were at the outdoor area with a group of children. The incident was reported to the parent of the child#1 and Coordinator. This is a Type A citation and repeat violation. This poses a immediately health and safety risk to children in care. 8 report date and maintain copy of lic9924 on children's file and copy of the form lic9224 will be email or mail to LPA no later than 02/3/2023 via email.

On 9/1/2022 child #1 was left at the big yard at playhouse without supervision. Child#1 was brought to the classroom 10-12minutes later and Staff were not aware that child#1 was left alone at the big yard at the playhouse.This is a Type A citation and poses a immediately health and safety risk to children in care. Coordinator was further instructed to complete this same process for all children who enroll in the facility within 12 months of this report date and maintain copy of lic9924 on children's file.
