101229: Responsibility for Providing Care and Supervision: "(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time..." This requirement is not met by evidence by: 1 Facility must notify parents of this incident. Written report of this incident must be submitted to LPA Tapia-Mandujano by end of day 7/15/26. Based on observation and interview, the facility did not comply with the section cited above as it was determined during a complaint investigation that a child was left unattended in the outdoor yard for approximately 3 minutes. This poses an immediate health, safety or personal rights risk to persons in care. 8 Facility will also conduct a training of transitioning and supervision policy with all staff. Agenda and ackowledgement of staff must be submitted to the LPA Tapia-Mandujano by Agust 7th, 2026.

Based on observation and interview, the facility did not comply with the section cited above as there is a storage bin outside that is broken and acessible to children in care. This poses a potential health, safety or personal rights risk to persons in care. 8 Written acknoweledgment statement from site director understanding that potential hazardous materials should not be accesible to children shall be submitted to LPA Tapia-Mandujano by 7/17/26.
