Based on record review, one outlet at the facility contained very high lead exceedance greater than 5.5 ppb. This poses a potential health, safety or personal rights risk to persons in care. 8 Director will notify all parents/authorized representatives of any corrective action that will be taken at the facilty. Director will come up with a plan to have the valve corrected or replaced and have the facility re-tested once the valve has been replaced.
