Based on observation, the licensee did not comply with the section cited above. Upon inspection at 8:25 am one staff was supervising 13 children in classroom # 2. At 8:30 am S#1 arrived to the classroom #2. This poses an immediate health, safety or personal rights risk to persons in care.
Based on records reviewed 3 of 9 (S1, S3, and S4) staff TB tests were outside the time frame required by the regulation and 1 of 9 (S2) staff did not have proof of TB test and physical on file which poses a potential risk to the health and safety of children in care.
Based on LPAs observations, the licensee failed to keep the childcare center in good repair at all times to ensure the safety and well-being of children, employees and visitors which poses a potential risk to the health and safety of children in care.
