Facility Admin understands the importance of providing care and supervision for children in care at all time. Facility Admin will conduct a training session with staff regarding the importance of active supervision and accountability of children. On 04/08/2025 children from room 6 were transitioning to the cafiteria from the playground area. A child was left behind unattened on the playground for a period of 5-10 minutes before they were discoved by school staff. This is an immediate health, safety and personal rights risk. 8 Documentation of training, staff signatures of training completed will need to be submitted to LPA on or before the date of correction, 04/15/2025.
