Based on record review, the licensee did not comply with the section cited abovewhich poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analyst (LPA) observed that the the three staff on site did not have their immunization records on file as well as a Health Screening Report. In addition, staff #1 was missing her educational record. Staff #3 was missing Mandated Reporter Training.
Based on interviews and video footage, child #1 walked outside of the gated preschool area and was without adult supervision for approximately ninety (90) seconds. This poses an immediate health, safety, or personal rights risk to children in care.

During employee record review, Licensing Program Analyst (LPA) observed that two substitute staff members were missing Mandated Reporter Training for Child Care Providers.
Based on observation, the licensee did not comply with the section cited above. The playground structure in the play area has plastic that has peeled off, leaving sharp edges, and exposed metal which poses a potential health, safety or personal rights risk to persons in care.
