Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed that the main blue carpet in Clasrooms 1 and 2 had dark marks and staining throughout. LPA observed some dead insects/flies along the bottom of the windows/window sills in Room 2. LPA also observed some ants on the cubbies near the front entrance in Room 1 and around the outside of the door in Room 2. Lastly, LPA observed a strong odor of urine coming from the bathroom in Room 1.

LPA was informed by Staff #3 that adult is not fingerprint cleared or a volunteer. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance.
