On 05/18/2026, the facility removed the play structure that was determined to not be age appropriate by a Certified Playground Safety Inspector. On 05/20/2026, the facility was determined to be in compliance with regulation by a Certified Playground Safety Inspector. A child was injured on a play structure present at the facility that was determined to not be age appropriate by a Certified Playground Safety Inspector, which posed an immediate risk to the health, safety, or personal rights to children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 By 09/09/2019, Director will submit a written plan of action on ways facility will ensure visual supervision of children in care at all times. This requirement was not met as evidence: child was able to interact with another child inappropriately. 8
